Bringing these forms with you will save time and help us to serve you better.

Our payment policy is a necessary part of assuring the resources to maintain our practice. Fees for services are based on the cost of the procedures and the amount of professional time involved. Our Billing Specialist will be glad to talk with you about the cost of appointments and specific procedures.

Although we file your insurance as a courtesy to you, it is our policy to allow 30 days for insurance settlement. While we will help with any problems that may arise, you are ultimately responsible for the charges. You are responsible for any required co-payment, deductible, and unpaid balance. In times of medical emergencies or extended medical problems you may establish a payment schedule to pay your bill. You may call our Billing Specialist to make arrangements for payments at 910.295.2945.
